The Apolipoprotein E (ApoE) gene represents the most important and well-established genetic risk factor for the development of late onset Alzheimer's Disease (AD). Using neuroimaging, we investigated whether the presence of single ε4 allele is associated with a differential atrophy in the hippocampal subfields. A total of 326 cognitively healthy and AD subjects underwent T1-weighted structural MRI imaging. Each raw images were initially pre-processed for quality assurance. Automated image processing pipeline and hippocampal subfield segmentation followed by the resultant volumes quality control were performed using Freesurfer software suite. ApoE genotyping were determined for each subject. There were significant differences in hippocampal subfields between the carriers and non-carriers of the ApoE ε4 only in AD subjects. Though, certain subfields did not show statistically significant differences. The volumes of the subfield showed an asymmetrical pattern of atrophy in the two hemispheres between the carriers and non-carriers of the ApoE ε4. Overall, the volumes of the AD were severely atrophied than the cognitively normal subjects. Findings from our study indicate that volumetric differences in hippocampal subfields between ApoE ε4 carriers and non-carriers are severe in the Alzheimer subjects. Additionally, suggesting that the pathophysiological mechanisms involved in the cognitively normal and abnormal aging might be different. The ApoE ε4 allele appears to have a greater deleterious effect on the hippocampal subfield pathology of the AD than the cognitively normal subjects.
