OP01.01: Is simulation training only for inexperienced trainees? Exploring the effects of simulation‐based ultrasound training on obstetricians' diagnostic accuracy

To evaluate the effect of simulation-based obstetric ultrasound training on the diagnostic accuracy of fetal weight estimation by ultrasound for obstetricians with different levels of clinical experience. The study was a randomised pre-post-test study, conducted at Copenhagen Academy for Medical Education and Simulation and six obstetric departments in Denmark, from March 2016 to January 2018. Participants were medical doctors from obstetric departments with different levels of clinical experience. Participants were randomised 1:1 to either an intervention group or a control group. In the intervention group participants trained using the virtual reality ultrasound simulator “Scantrainer” from Medaphor (Cardiff, UK) in addition to their clinical training until they passed a previously validated test. Finally, they trained using a physical mannikin (Kyoto Kagaku SPACEFAN-ST, Kyoto Japan) until they reached a passing score on the OSAUS-scale. Participants randomized to the control group continued their clinical work as normally. At baseline and approximately 2.5 months later, participants estimated fetal weight by ultrasound in two pregnant women at term. Birth weights were recorded and compared to the estimated fetal weight. The intervention group showed a reduction in their diagnostic error by -31.9% (95% CI -50.1– -6.9) (p = 0.02), whereas the control group did not reduce diagnostic error significantly (13.1% (95% CI, -17.9 – 55.9), p = 0.45). The difference between groups was significant (p = 0.02). There was no effect of clinical experience (p = 0.54) on the effect of the intervention. Simulation-based obstetric ultrasound training improved the diagnostic accuracy of fetal weight estimation by ultrasound in novices and experienced obstetricians alike. Simulation-based training may help young obstetric trainees prepare for future clinical practice, but it may also act as a means of maintaining skill in more experienced clinicians.
