This chapter discusses the more recognized heterogeneities of the red cells from patients homozygous for HbS, particularly those resulting from the heterogeneity of red cell density or mean corpuscular hemoglobin concentration (MCHC). MCHC has a strong effect on the cell which contains hemoglobin S because of the extreme sensitivity of both the rate and extent of polymerization of deoxy HbS to the intracellular concentration of HbS. The equilibrium and kinetics of the polymerization of HbS are highly dependent on the conformational state of the HbS molecule. Drastic reduction of the ionic strength inhibits considerably the tendency of HbS to polymerize, which is rapidly restored when lower than physiologic concentrations of salts are added. At low hemoglobin concentrations, that is significantly below the concentration found in red cells, HbS has a completely normal oxygen binding curve and completely normal on and off constants for several ligands.
