Background: Patients with autoimmune rheumatic diseases are at an increased risk of cardiovascular vascular disease (CVD). Both SLE and RA are characterised by immune dysfunction and, particularly for RA. iNKT cells are a small population of immune cells. Previous work at by this group at UCL with SLE patients who had plaque but were asymptomatic for CVD were found to have an activated iNKT cell profile compared to SLE patients with no plaque. Methods: We investigated the role of iNKT cells in 8 plaque (RA-P) and 8 non-plaque RA (RA-NP) patients asymptomatic for CVD. Patients were matched by age, sex and ethnicity. Patients had carotid US scans by a collaborator and serum samples were sent to UCL. PBMCs were cultured from healthy donors with serum from RA-P, RA-NP and healthy controls over 7 days. Results: We found that iNKT frequency and activation was reduced in RA patients compared to healthys. No difference in iNKT frequency was seen in the RA-P and RA-NP group and analysis of iNKT frequency in the P Vs NP groups did not show any significant statistical correlations with disease activity, treatments, carotid intimal media thickness, serum lipids and traditional CVD risk factors. Conclusion: In conclusion, this study shows that the iNKT cell profile differs in asymptomatic CVD patients with RA compared to SLE patients. This may be due to the small sample of RA CVD patients (n = 8) compared to the SLE CVD patients (n = 36). The differences may also be explained by the fact that the RA patient were significantly older (median age 63 vs 55 in the SLE group) with a greater prevalence of traditional CVD risk factors.
