Analysis of the solitary pulmonary nodule after surgical resection in patients using 18F-FDG integrated PET-CT scan at preoperative work-up

Objetives: To analyze after surgical resection the characteristics of patients operated on for solitary pulmonary nodule(SPN) up to 3cm in size with preoperative 18F-FDG integrated PET-CT scan and without diagnosis. Methodology: retrospective study of patients operated in the last 10 years. Dependent variable was histology result (benign vs malignant) of SPN. Variables of the study: age, sex, uptake on PET-CT scan, maximal standard uptake value(maxSUV), tobacco, chronic obstructive pulmonary disease(COPD), previous malignant disease, nodule size. Results: 305 patients were included in the study, 225(73.8%)male, 80(26.2%)women, mean age=63.9(rank 29-86), mean size of SPN=1.68cm(s.d. 0.65cm), benign=46(15.1%), malignant=258(84.6%), COPD=50.8%, previous malignancy=172 cases(56.4%), smoking=250 cases(82.0%), positive PET=280 cases(91.8%), median maxSUV=3.4(rank 0-20.7). Binary logistic regression model by backward steps showed age, maxSUV, previous malignant disease, and female sex were statistically significant risk factors (p<0.05, table-1). Area under the curve was 0.804; for cutoff point 0.536 sensitibity and specificity were 97.2% and 34.8% respectively. Respect to maxSUV, area under the curve was 0.694; for cutoff point 1.1 sensitibity and specificity were 90.2% and 30.4% respectively. Conclusions: age, SUVmax, medical history of previous malignant disease, and female were independent risks factors in our analysis of patients operated on for SPN.
