Background: It is well established that long lasting atrial fibrillation (AF) is a hypercoagulable state. Despite its short duration, paroxysmal AF (PAF) carries the same embolic potential. However, its haemostasis is poorly studied and data are even contradictory. Von Willebrand factor (vWF) plays a crucial dual role in haemostasis by promoting platelet adhesion on exposed endothelium and functioning as carrier protein for the key factor in the intrinsic clotting cascade, namely factor VIII (FVIII). Purpose: To examine plasma levels and activity of FVIII and vWF still in the first twenty-four hours of the clinical presentation of PAF. Methods: We selected consecutively 51 non-anticoagulated patients (26 men, 25 women, mean age 59.84±1.60) with PAF episode duration ≤ twenty-four hours and 52 controls (26 men, 26 women, mean age 59.50±1.46 years) corresponding in gender, accompanying diseases and conducted treatment. The hemostasis markers were examined using enzyme-linked immunoassays. Results: All patients were hospitalized between the 2nd and 24th hours after the onset of the arrhythmia (mean 8.14±0.74 hours). FVIII plasma levels and activity in the patient group were significantly higher (93.85%±2.93% vs 107.52%±4.36%, p<0.05, 109.73%±4.90% vs. 200.03%±11.11%, p<0.001, respectively). We also found elevated vWF levels and activity (119.53%±6.12% vs 110.84%±3.14%, p<0.001, 110.80%±5.14% vs. 200.92%±12.45%, p<0.001, respectively) in the PAF group.
