The aims of this study were to investigate the changes in the thicknesses of quadriceps femoris muscle during pregnancy and puerperium as a longitudinal study and to evaluate the effect of bed rest on thicknesses of quadriceps femoris muscle in pregnant and postpartum women. (Study1) The study population comprised 23 Japanese pregnant women. All women were recruited for the longitudinal study from the outpatients clinic of the Department of Obstetrics and Gynecology, Tokushima University Hospital. Muscle thicknesses of six sites: proximal (RFp), intermediate (RFi) and distal part (RFd) of rectus femoris and proximal (VIp), intermediate (VIi) and distal part (VId) of vastus intermedius were measured using ultrasonography at 12, 26, 30 and 35 weeks of pregnancy and 4 days and 1 month postpartum. (Study2) We recruited 11 pregnant women who were treated by bed rest before 30 weeks for threatened premature delivery and compared the muscle thicknesses in these women with those in 23 normal pregnant women. Muscle thicknesses of same six sites as above: RFp, RFi, RFd, VIp, VIi and VId were measured using ultrasonography at 30 and 35 weeks of pregnancy and 4 days and 1 month postpartum. The study was approved by the ethics committee of Tokushima University Hospital (number 2037–1), and informed consent was obtained from all participants. (Study1) Muscle thicknesses of RFi, VIp, VIi and VId increased gradually during pregnancy and showed a peak at 35 weeks of pregnancy, followed by a decrease in the postpartum period. (Study2) Muscle thicknesses of VIp and VIi at 35 weeks of pregnancy in women with bed rest were significantly (p < 0.05) lower than those in normal pregnant women. In the postpartum period, there was no significant difference between the muscle thicknesses of all 6 sites in normal pregnant women and women with bed rest during pregnancy. The thickness of quadriceps femoris muscle increases in normal pregnancy, whereas immobilisation due to bed rest during pregnancy suppresses these changes.
