Papel de la melatonina en el estado oxidativo del líquido folicular y su relación con la calidad de los oocitos en mujeres con problemas de fertilidad

En particular, actua neutralizando radicales en procesos como la gametogenesis, fecundacion, desarrollo fetal y parto. Ademas se ha descrito su secrecion en ovario y placenta. El Objetivo de la presente Tesis de Doctorado es estudiar la relacion entre el estado oxidativo de pacientes sometidos a tecnicas de reproduccion asistida y el efecto de la administracion de melatonina sobre la calidad ovocitaria y embriones, y por tanto sobre el exito de las tecnicas de reproduccion asistida. Para ello se administro a un grupo de mujeres infertiles diferentes dosis de melatonina (3 mg o 6 mg/dia) durante 45 dias previos a la puncion folicular, ademas de contar con un grupo de pacientes fertiles como referencia. Los resultados indican que las pacientes infertiles tienen niveles inferiores de melatonina circulante con respecto a las fertiles. Este tratamiento indujo niveles de melatonina en LF incrementados en mujeres tratadas con melatonina con respecto a mujeres infertiles no tratadas. Dicho aumento se observo de forma paralela en la capacidad antioxidante total de estas pacientes que revirtio los marcadores de dano oxidativo en el liquido folicular. La administracion de melatonina aumento el numero de cumulos recuperados, el ratio de ovocitos maduros, el ratio de ovocitos fecundados. Todo esto se reflejo en un incremento de embriones transferibles, y aunque no alcanzando los niveles de pacientes fertiles, se materializo en una mayor tasa de embarazos. In particular, it acts by neutralizing radicals in processes such as gametogenesis, fertilization, fetal development and childbirth. In addition, its secretion has been described in ovary and placenta. The objective of this PhD Thesis is to study the relationship between the oxidative status of patients seeking assisted reproduction technologies (ART) and the effect of melatonin administration on oocyte quality and embryos, and therefore on the success of ART. For this purpose, different doses of melatonin (3 mg or 6 mg/day) were administered to a group of infertile women for 45 days prior to follicular puncture, in addition to having a group of fertile patients as reference. The results indicate that infertile patients have lower levels of circulating melatonin than fertile ones. This treatment induced increased melatonin levels in follicular fluid in women treated with melatonin with respect to untreated infertile women. This increase was parallel to the total antioxidant capacity of these patients that reversed the markers of oxidative damage in the follicular fluid. The administration of melatonin increased the number of follicles recovered, the ratio of mature oocytes and the ratio of fertilized oocytes. All in all, this was reflected in an increase in transferable embryos, despite not reaching the levels of fertile patients, the treatment with melatonin induced a higher rate of pregnancies in infertile patients.
