Background: Several studies have demonstrated that remote management (RM) reduces the total number of in-office visits, without negative effects on patient outcomes. Nevertheless, some workflow changes are required in daily practice, which requires new definition of roles and responsibilities, within the hospital clinical teams. Purpose: To evaluate the efficacy and safety of an external remote monitoring center (ERMC) to remotely manage and follow-up patients wearing cardiac implantable electronic devices (CIED). Methods: A flow-chart describing timings, role and responsibilities has been agreed between hospital physicians and the ERMC staff. With the aim of effectively reviewing transmissions and managing all possible clinical and technical relevant events, a transmission code classification was defined by the hospital physicians to prioritise device clinical and technical conditions, and the associated actions (e-mails, phone contacts and specific comments on the RM platform websites). In case of missed transmission or disconnected monitors, the EMRC staff contacted the patient by phone for investigation and troubleshooting and, if a technical issue was highlighted, the ERMC team would involve the team responsible to fix it. A comparison with the standard RM, where patients' transmissions were reviewed directly by the hospital staff without a pre-defined protocol is also presented.
