BACKGROUND: Iron deficiency (ID) is the most common nutritional disorder worldwide.ID not only causes anemia (IDA), but it affects virtually all organ systems and can cause serious health problems in children which are preventable with appropriate nutrition.The WHO considers the prevalence of anemia in excess of 5% to be a mild public health problem.In Canada, there are no surveillance programs for ID, and data is limited to small studies that have found rates of up to 8.4% in middleclass urban and 79% in high risk populations.OBJECTIVE: Our objective is to estimate the local prevalence of ID and IDA in children.We hypothesize that the prevalence of IDA will exceed 5%, and merit prospective study of prevalence in our population.METHOD: Data was acquired from our health region's laboratory database for children ages 12-59 months who had a complete blood count (CBC) done May 1st 2002-June 30th 2008.Though the group is not a random sample, the data provides an estimate of prevalence of ID/IDA in our community.ID was defined as mean cell volume (MCV)<70fL and red cell distribution width (RDW)>15.6%and IDA as haemoglobin (Hb)<110g/L with ID.Descriptive statistics for Hb, MCV, RDW and prevalence of ID and IDA were calculated for the group and age ranges using Stata for statistical analysis.Ethics approval was attained for this study.RESULTS: 32,069 observations were included.The mean age was 31 months and 53.9% were males ("Figure"-Table 1).demonstrates the trend to increase in hemoglobin and MCV with age, and a decrease in RDW with age."Figure "-Table 2 demonstrates that there is a statistically significant decrease in prevalence of anemia, ID and IDA as age increases.ID and IDA peaked at 19 months (11.65% and 7.69% respectively.Groups 1 and 2 had prevalence rates for ID and IDA exceeding 5%.CONCLUSION: These data demonstrates that anemia and ID remain mild public health problems in our population.We recommend that a prospective study be undertaken to further define this problem, develop targeted interventions, and plan for ongoing surveillance.ID results in significant preventable health problems for children, and requires immediate attention.
