Elevated levels of alveolar nitric oxide in patients with inflammatory bowel disease

Introduction: Inflammatory bowel disease (IBD) is associated with extraintestinal complications, among which pulmonary manifestations are rarely considered. Exhaled Nitric oxide (eNO), an established biomarker of airway inflammation , may be useful in investigating these overlooked manifestations. Aim: To examine airway inflammation in IBD and investigate whether eNO can be used as surrogate marker for disease activity in IBD. Methods: Alveolar NO (Calv), bronchial NO (JNO) and Fractional Exhaled NO (FeNO) were measured in 29 IBD patients (19 with Crohn9s Disease and 10 with Ulcerative Colitis) and in 33 healthy controls. Previously, spirometry, medical history and Rast tests were used to exclude the asthmatics from both groups. Disease activity was measured in Crohn9s patients using the CDAI score and in all IBD patients using CRP measurements. Results: Patients with IBD were of mean age 42±15 years. Mean Calv, JNO and FeNO of the IBD group vs the control group were 5.18±4.5 vs2.17±1.17 ppb, p+0.0005, 2387±1587 vs 2144±1277 pl/s.p=0.5, and 35.07±20.5 vs 31.06±18.9 ppb, p=0.4, respectively. There was no correlation between age and Calv measurements of the IBD (p=0.6) or control subjects (p=0.25). There was also no significant correlation between Calv measurements and both the CDAI scorings (p=0.6, r=-0.14) and CRP levels (p=0.25, r=0.34), respectively. Conclusions: There seems to be an increase only in Calv levels and not in JNO and FeNO in IBD patients compared to the general population. This can be attributed to peripheral inflammation in the lungs of IBD patients. More conclusions regarding the relationship between eNO levels and disease activity are to be drawn at the end of this ongoing study.
