Subcutaneous mycobacteriosis in renal transplant patients

Nontuberculous mycobacterial infections are caused by opportunistic organisms that primarily affect immunocompromised individuals. While these infections usually produce systemic manifestations, subcutaneous and/or cutaneous mycobacteriosis, either in association with disseminated disease or as an isolated finding, does occur and may be the only manifestation of the disease in the transplant population. The presence of a new subcutaneous mass on a patient receiving immunosuppression should be evaluated by biopsy for histologic examination (to rule out malignancy) and culture and sensitivity. Once the diagnosis of mycobacterial disease is made, and disseminated disease ruled out, initial treatment by reduction of immunosuppressant dosages should be instituted with close observation. Development of disseminated disease and/or allograft failure should be treated by the reduction of immunosuppressants as well as with antimicrobial agents to which the organism is susceptible. This report describes two renal allograft recipients who presented with biopsy‐proven, isolated, nontubcrculous, subcutaneous mycobacteriosis while receiving conventional azathioprine and prednisone therapy. The literature concerning subcutaneous and cutaneous mycobacteriosis in renal allograft recipients is reviewed.
