Differences in management and outcomes between young and elderly patients with acute aortic dissection type A: Results from the German Registry for Acute Aortic Dissection type A (GERAADA)

Objective: The recent improvements in treating patients with acute aortic dissection type A (AADA) has led to more elderly patients undergoing emergency surgery for AADA. The aim of this study was to analyse clinical characteristics and surgical outcomes in young and elderly patients with AADA. Methods: Between 7/2006 and 6/2009, 44 centres participating in the German Registry for Acute Aortic Dissection Type A (GERAADA) reported a total of 1558 patients. 1094 patients were younger than 70 years, 464 patients were 70 years and older. We compared both groups' clinical features and 30-day post-operative events. Results: On admission, 25% (n=115) of the elderly patients presented cardiac tamponade compared to 17% (n=183) of the younger patients (p<0.001). Aortic dissection extended to the supra-aortic vessels (38% vs. 28%, p<0.001) and abdominal aorta (38% vs. 29%, p<0.001) more frequently in the younger patients. The younger patients were more likely to undergo complex surgery such as composite graft implantation (24% vs. 14%, p<0.0001), David operation (9% vs. 3%, p<0.0001) and replacement of the aortic arch (18% vs. 13%, p<0.05). Mean stay in the intensive care unit was 7 days in the younger, and 11 days in the older group (p<0.0001). The younger patients 30-day postoperative mortality was 15%(n=158), whereas the elderly patients' was 20% (n=92; p<0.0001). Conclusions: Elderly patients undergoing emergency surgery for AADA experience acceptable mortality. Recent technical improvements in emergency surgery for AADA have resulted in acceptable outcomes, making the age factor more relative.
