P1‐057: Screening of Alzheimer's disease in elderly diabetes using self‐reported questionnaire and physical parameters

Diabetes has been proposed to increase the risk of Alzheimer's disease (AD). AD often remains unrecognized or misdiagnosed at the earliest stages in the treatment of diabetes by primary-care physicians. For detection of AD, information on recent impairment of memory and activities of daily living (ADL) from the caregivers is important, but such interviews are not always accessible in the ambulatory care of diabetes. The aim of this study is to develop a brief self-reported questionnaire for screening AD composed of cognitive tests and succinct physical parameters. 496 diabetic elderly were recruited in Kobe university hospital. For screening of AD, we used a computer-based screening program. Accordingly, 25 were diagnosed as possible AD according the criteria from NINCDS-ADRDA and 380 were normal in cognitive function. Participants were asked to fill out a questionnaire regarding memory disturbance and instrumental ADL and investigated for temporal orientation. Information on diabetes, therapy and vascular complications was obtained from clinical chart. A multivariate logistic regression was used to examine the independent association of these clinical variables with AD. Comparing with control group, AD group was characterized by higher age, lower education, lower diastolic blood pressure, and impaired temporal orientation. Drug compliance, preparing meal, shopping, financial management, and habitual use of note were significantly impaired in AD. Backward regression identified six factors were associated with AD (age, diastolic blood pressure, drug compliance, preparing meals, habitual use of note, and temporal orientation). ROC analysis revealed the satisfactory discrimination of AD among diabetic elderly (sensitivity: 95.0% and specificity: 86.6%). Our self-reported questionnaire consisting of physical parameters, instrumental ADL, and temporal orientation, was available for screening of AD among elderly diabetes. However, the clinical usefulness of this index needs to be validated by large-scale prospective studies.
