Results: Univariate analysis by means of the Cox Proportional Hazard Regressions revealed that 15 factors were prognostic predictor including gamma glutamyl transferase, Alkaline phosphatase, preoperative total bilirubin level, liver cirrhosis, Child-Pugh classification, postoperative ascites, time of operation, preoperative total bilirubin level, tumor rupture, TNM staging, Edmonson grade tumor size, HLA-DR alpha, b-catenin and DNA methyltransferase 1 P < 0.05.In a multivariable Cox regression model analysis found that the independent significant parameters were TNM staging(RR: 1.849, 95%:1.293-2.645)preoperative total bilirubin level (RR: 2.539, 95%:1.430-4.509),intraoperative blood loss(RR: 1.987, 95%:1.083-3.643),HLA-DR alpha (RR: 0.350, 95%:0.198-0.616)and DNA methyltransferase 1 (RR: 2.570, 95%:1.401-4.715).These variables can be used to calculate a risk score (R) for HCC patients after resection and were developed a HCC prognostic estimation formula (HPEF).ROC curve revealed that the accuracy of HPEF is the best among the five prognostic estimation systems. Conclusions: The results confirmed that it is feasible to develop an estimation system to predict prognosis in HCC patients after resection, based on independent prognostic biomarkers and clinical factors. Our system provide a more precise prognosis than traditional system for individuals who undergoing hepatic resection.
