Material and methods: This study was conducted in two large university centers.We collected data on first trimester ultrasound in pregnancies obtained by in-vitro fertilization (IVF).Crown-rump length (CRL) at 11-14 weeks was expressed as Z score of the expected CRL for the actual gestational age, based on the date of oocyte retrieval.At term, birth weight was expressed as Z score of the expected birth weight adjusted for gestational age and fetal gender.The relationship between CRL Z scores, maternal co-variates and BW Z scores was investigated.Results: Data in the first trimester and at term were available in 321 pregnancies.Mean (+/-SD) maternal age, gestational age at delivery and BW were 33.6(4.1)years, 39.2(2.0)weeks' and 3240 (544) grams respectively.BW Z scores were strongly positively associated with CRL Z scores (p=0.001) and maternal body mass index (BMI) (p=0.008).Once adjusted on BMI, each decrease of one unit in CRL Z scores was associated with a two-fold increase in the risk of having a growth restricted baby (p<10-4).Conclusion: CRL Z scores are strongly associated with BW Z scores, suggesting that fetal growth might express as early as in the first trimester.Better understanding of first trimester growth patterns may help in early identifying high risk patients who may deserve particular management and follow up.
