Objectives: We developed a web-based database to record peri-operative and follow-up data on patients who had undergone surgery for type-A aortic dissection with the aim to learn from data analysis how to improve peri-operative management and surgical treatment. We also aimed to identify risk-score parameters affecting patient outcome. Methods: All cardiac centres in Germany, Austria, and Switzerland have online access to the registry on the GSTCVS homepage. Data on pre- and intra-operative status, post-operative complications, mid-term results, and circumstances of death have been recorded. Data analysis was undertaken between July 1, 2006 and June 30, 2009. Results: Over a three-year period, 44 centres entered data on 1,610 patients. Data were complete and evaluated in 1,558 cases. Examples of pre-, intra-, and post-operative data are illustrated in Table 1. Tab.1 Pre-, intra-, and post-operative data PRE-OPERATIVE DATA patients (%) mortality (%) INTRA-OPERATIVE DATA patients (%) mortality (%) POST-OPERATIVE DATA patients (%) mortality (%) hemodynamic instability 856 (55) 176 (21) ascending replacement 1053 (68) 167 (16) bleeding complications 348 (22) 78 (22) malperfusion 486 (31) 121 (25) composite graft 332 (21) 63 (19) reoperation for bleeding 212 (14) 45 (21) neurologic events 308 (20) 94 (31) David-procedure 107 (7) 10 (8) new neurologic events 160 (10) 32 (20) overall 30-day-mortality 250 (16) Yacoub- procedure 32 (2) 5 (16) new malperfusion 107 (7) 32 (30) Conclusions: These first three years have led to the largest registry for acute type-A aortic dissection worldwide. After analysing the data we can perceive initial trends, and this information is made available to all the participating centres. All will benefit from this analysed data by optimising their treatment regimes for patients with acute type-A aortic dissection.
