Turbodynomic ventricular assist pumps, due to their otherwise odvontogeous lock of thrombogenic volves, hove been criticized for ossocioted hemodynomic risk during pump stoppoge. A iypicolly connutoted VAD, intended to ougrneht aortic flow and orteriol pressure when operating normally, may result in considerable retrograde flow if the impeller should stop spinning. lnvestigotors concerned with this hozord hove proposed check volves, boloon occluders, and other desporote meosures to reduce mortolity risk. However, others hove observed sotisfoctory perfusion during pump stoppoge. This experimental and theoreticol study demonstrates the influence of the connulo design upon regurgitont flow. It is found thot the combined resistonce ond fluid dynomic inertonce of the pump and connulo rnoy be tuned to minimize regurgitont flow, and even ollow sotisfoctory perfusion, provided there exists residuol ventricular function. A computer simulation ond in-vitro study, supported with in- vivo evidence, provide general relotionships thot may be applied to any turbodynornic VAD. Addtitionol clinicol interventions, porticulorly administration of chronotropic drugs, hove been simulated to opiimize perfusion, suggesting on optimol emergency procedure thot would improve survivol in the event of catostrophic pump foilure.Table
